This site is designed to assist both you and your family in making choices which best meet your needs for the upcoming year. As you are undoubtedly aware, healthcare costs have risen at unprecedented rates over the last few years. Tonkawa and Native Lights Casino continues to evaluate and balance the rising cost of healthcare while continuing to provide employees with benefit options designed to promote wellness and provide protection for you and your family at an affordable price.
HealthSmart will still be administering our medical claims this year, and HSRx will still be administering our prescription claims. We are still using the Blue Cross & Blue Shield of Illinois PPO Network, so there is no change to the providers that you will have access to.
While most organizations are reducing benefits and increasing premiums we are excited to announce that
once again, we will not be making any changes to our health, dental or vision benefits OR employee contributions for those plans for 2024. We will continue to offer additional voluntary benefits offered through Hartford and Colonial Life. These benefits include voluntary accident, critical illness and universal life insurance.
This guide highlights the options available to you and explains how to enroll in your 2024 benefits. Please review this enrollment guide carefully. For specific details of any of the plans offered, please contact your Human Resources Department for a copy of the Summary Plan Description.
The open enrollment period runs from November 13, 2023 through November 27, 2023. The benefits you elect during open enrollment will be effective from January 1, 2024 through December 31, 2024.
If you are a full-time employee of the Tonkawa or Native Lights Casino (regularly scheduled to work 40 hours but not less than 30 hours per week) you and your eligible dependents are eligible to enroll in the benefits described on this site. New hires will become eligible for benefits on the 1st of the month following 60 days of full-time employment.
Open enrollment is the time to make changes to your current elections that may include adding/dropping coverage, adding/dropping dependents, or increasing/decreasing coverage. Only employees new to the coverage or those wishing to make changes will be required to complete a new enrollment form. All completed enrollment forms must be turned in to your Human Resources Department by November 27, 2023.
Unless you have a qualified change in status, you cannot make changes to the benefits you elect until the next open enrollment period. Qualified changes in status include: marriage, divorce, legal separation, birth or adoption of a child, change in child’s dependent status, death of spouse, child or other qualified dependent, change in residence due to an employment transfer for you or your spouse, or losing or gaining coverage elsewhere. You have 30 days from a qualifying event to make changes to your current coverage.
