Frequently Asked Questions

What changes can be made effective January 1, 2025? 

  • Enroll or terminate individual and/or dependent coverage in the Medical/Dental/Vision plans. 
  • Apply for Voluntary Life/AD&D coverage or change your current elections. 


When do the changes go into effect? 

  • All changes will be effective January 1, 2025. 


When are the forms due and where do I return them? 

  • Forms must be completed and returned to the Human Resources Department to enroll or change coverage. 


Will I get a new ID card? 

  • You will receive a new ID card for the Medical coverage even if you make no changes. 


Can I make changes to my elections during the middle of the plan year? 

  • Employees cannot make changes to their elections during the plan year (such as dropping coverage or adding a dependent) unless you have a Qualifying Event. 


Does Luminare offer on-line access? 

  • Yes – log in to www.Luminare.com and find information about health management tools and resources that can save you money and help you manage your health.
